1099 is a tax for. But not teally relevent to the issue. We have a form that contains cells for entering date, job done, pay rate for job, etc. Need to create a form for this.

If you want to issue an invoice for services provided, the easiest and simplest option is to create a service and place this on invoices (the generic description can be extended to capture the particular data you rendered).
A somewhat more complex scenario involves the creation of a project. There you can set up tasks and record the work performed. From that you can easily create intervention documents and detailed invoices with all the information you mentioned just with the click of a button. Conversion between recorded time and cost is done via a generic service for that job.
